This Privacy Policy describes how SNS Health and Wellness Center ("SNS," "we," "us," or "our") handles information collected through our website, online forms, booking tools, payment pages, communications, and related digital services. It does not replace clinical consent forms, laboratory notices, employer testing authorizations, or our HIPAA Notice of Privacy Practices.
Our services are intended for adults age 18 or older. We do not knowingly offer website services to children or knowingly collect personal information from children through the website.

Appointment and inquiry information, including the service requested, preferred appointment time, referral source, and messages you send us.

Employment, occupational, and testing information when needed for DOT physicals, drug testing, employer-sponsored services, chain-of-custody procedures, or related compliance activities.
Transaction information associated with purchases or appointments. Payments are processed through Square or another payment processor; SNS generally does not receive or store your complete payment-card number.
Device and usage information, such as IP address, browser type, device identifiers, pages viewed, referring pages, approximate location, cookies, and similar technologies.
Marketing and communications information, including whether you opened a message, clicked a link, subscribed, opted out, or contacted us through a campaign.
3. How We Collect Information
Directly from you when you complete a form, book an appointment, make a payment, call, email, text, subscribe, or use a secure patient platform.
From service providers that support our website, scheduling, communications, payments, secure record collection, laboratories, and business operations.
From employers, third-party administrators, attorneys, accredited representatives, veteran organizations, referral sources, or other persons when you authorize the disclosure or when permitted by law.

Laboratories, collection sites, medical professionals, consultants, employers, third-party administrators, attorneys, VA-accredited representatives, or government agencies when needed for a requested service, authorized by you, or permitted or required by law.
Professional advisers, regulators, licensing bodies, law enforcement, courts, or other parties when disclosure is legally required or reasonably necessary to protect rights, safety, property, or the integrity of our services.
A successor or transaction participant in connection with a merger, financing, reorganization, sale, or transfer of some or all of our business, subject to applicable confidentiality and healthcare-privacy requirements.
SNS does not sell medical records. SNS does not authorize advertising partners to use protected health information for targeted advertising. Reply STOP to a marketing text to opt out and HELP for assistance. You may unsubscribe from promotional email using the link in the message or by contacting us. Opting out of marketing does not stop necessary transactional, safety, or service communications where legally permitted.
Email and ordinary text messages may not be secure. Do not use them for emergencies or to transmit sensitive medical records unless SNS specifically authorizes the method. Call 911 or seek emergency care for a medical emergency.

SNS uses reasonable administrative, technical, and physical safeguards appropriate to the nature of the information. No website, email system, text service, or electronic transmission is completely secure, and we cannot guarantee absolute security. SNS provides in-person services only in Georgia. Virtual services may be available only in locations where the treating professional is authorized to practice and where the service can lawfully be delivered. Availability must be confirmed with SNS before booking; website access does not establish eligibility for care in a particular jurisdiction.
